This form is to register your child(ren) for participation in the Van Walleghem School Parent Council Lunch Program,

 

 

for the school year beginning in September 2008. It is essential that we receive completed registration forms for

 

 

all returning and new students by Friday, May 30, 2008.

Invoicing - for occasional Lunch Program use ONLY - students will be invoiced $1.75 per day.

 

 

 

 

 

(Note: If your child stays on average 3 days per week - the full time program is a more

 

 

 

 

economical choice). Families will be invoiced for lunch hours their children stay at school, including

 

 

 

hot lunch days, and extracurricular activities where lunch supervision is provided between 12 - 1:00 pm.
